Reservation Form - The purpose of this form is to reserve the week and the space needed for your church. Send this form in immediately with a $30.00 deposit per camper, including sponsors. Some deposits have already been received, but you will still need to return the form with your signature.
Registration Form - This form shall be completed with each camper's name and information (including sponsors) and mailed by March 27, 2009. The balance of the camp fee per person must be submitted to secure the space previously reserved. Unused space will be given to churches on the waiting list. There will be no refunds given after March 27, 2009. Needed space can be purchased from other churches attending the same week. Call the CrossTimbers Program Office (405-942-3800, ext. 4662) for that information.
Packing List for Campers - Packing for camp can be tricky so here's a list of what to bring. Feel free to revise the list as needed. Notice the items not to bring and discuss them with you group.
Health Form - This form must be completed with detailed information including Insurance and immunizations or a copy of the health insurance card (front and back) and a copy of the immunization record. Sponsors must also complete the health form with insurance information in case of an accident.
Medication Form - For the safety of each camper, all medication, prescription or non-prescription drugs will be held at the camp First Aid Station and administered by camp-approved, certified medical personnel, who are on duty 24 hours per day. Provide this form for parents to clearly communicate all medicine treatment needs. Sponsors should collect medicine before leaving for camp to check that informaton is provided clearly and completely.
